Fatty Liver Reversed on a 12-Week Plan with Guidance from the British Liver Trust
A FIB-4 score can turn a vague liver comment into a concrete risk check, using age, platelets, ALT, and AST. The British Liver Trust puts that score beside a 12-week push on liver fat, where weekly fasting glucose, alcohol abstinence, oily fish, and repeatable exercise carry much of the load.
Raised ALT on a routine blood panel and an incidental comment on an abdominal ultrasound are the usual routes into a fatty-liver diagnosis. A liver biopsy is an uncommon starting point for most people. The British Liver Trust runs a self-assessment questionnaire and publishes the scarring risk score that many GPs now use, FIB-4. It combines age, platelet count, and two liver enzymes into one figure. For people under 65, a result under 1.3 is generally reassuring. A result above 2.67 usually leads to referral. The 12-week plan sits below that risk-checking system, focused on reducing liver fat before fibrosis becomes the main issue.
Start with fasting glucose, then watch the scales
Hepatic fat and insulin tend to move together. When fat accumulates in the liver, the organ keeps releasing glucose into the blood overnight. That is why a fasting glucose result of 6.0 to 6.9 mmol/L, the NHS range for non-diabetic hyperglycaemia, so often appears alongside fatty liver on the same set of results.
The bathroom scale can be a poor early signal because water weight moves in jumps. Fasting glucose often responds sooner.
The daily target is the post-meal rise in blood sugar. Repeated spikes feed the fat-making pathway that stores triglyceride inside liver cells.
Two habits do most of the work during the 12 weeks. The first is replacing refined starch at the largest meal with a food that has more resistant structure. Pearl barley or lentils can take the place of white rice.
The second is a 10 to 15 minute walk within half an hour after eating. Working muscle draws circulating glucose out of the blood before the liver has to handle as much of it.
People using a cheap home meter once a week often see fasting glucose drift down before their weight changes. Expecting that order helps keep an early scale plateau from ruining the plan.
Alcohol is removed for the 12-week window
Even with non-alcoholic fatty liver disease, the British Liver Trust treats the 12 weeks as an alcohol-free period. The liver processes ethanol as a priority fuel, which slows fat oxidation while it clears the alcohol, and zero drinks removes that competing task.
Oily fish: portions first, capsules second
How much oily fish actually changes liver fat? The UK Scientific Advisory Committee on Nutrition sets a population target of one 140g portion of oily fish per week. The evidence around hepatic steatosis is clustered at a higher intake.
Trials using two to three portions weekly, delivering roughly 2 to 3 grams of combined EPA and DHA per day, are the ones showing reductions in liver fat on MRI-based measurement. Mackerel, sardines, herring, and salmon all fit. Tinned versions count fully and are budget-friendly.
Capsules look like the simple shortcut. Meta-analyses on omega-3 for fatty liver remain mixed on whether they reproduce the same results seen with whole fish.
The food version also brings protein and tends to replace a weaker plate choice, often processed meat or a takeaway. A practical structure for 12 weeks is three fish meals across the week: a tin of sardines on toast, a baked mackerel fillet, and a portion of salmon. The cost stays well under three pounds per serving when the salmon is frozen supermarket stock.
For someone who genuinely cannot eat fish, a 1g daily EPA/DHA capsule is the fallback. The food habit remains the stronger option in this plan.
Building aerobic volume without a gym
The NHS Couch to 5K programme fits almost exactly inside the 12-week frame. It has nine weeks of three sessions each, beginning with 60-second running intervals separated by walking, and ending with a continuous 30-minute run.
For fatty liver, the 5K distance itself carries less importance than the accumulated aerobic work. Moderate-intensity movement raises the rate at which the liver oxidises its own fat stores, and this effect can appear before body weight changes.
There is a common problem in this group. Many people with visceral fat also have deconditioned joints and a resting heart rate in the 80s, so the first week can feel tougher than the app suggests. Repeating a week still fits the goal.
The British Liver Trust and the NHS both frame progression around movement volume. Someone who does each week twice and reaches a 25-minute continuous effort by week 12 has captured most of the metabolic benefit.
Two short resistance sessions add another useful signal. Bodyweight squats and press-ups against a kitchen counter are enough to start. Added muscle acts as a glucose sink between meals.
In intervention literature, aerobic training plus resistance work performs better than either approach alone. Neither part requires a gym subscription.
Low-sodium meal prep that survives a real week
Sodium sits outside the direct cause of fatty liver, though it complicates the blood pressure and fluid picture that often travels with it. The highest-sodium ultra-processed foods are also dense in refined carbohydrate and fructose, two drivers of hepatic fat.
A batch-cook plan that lasts through a busy week starts with a base the fridge can hold for four days. Lentils or chickpeas can be simmered with no added salt, then bulked with tomatoes, garlic, and cumin.
Portioned containers become the default lunch. That removes the meal-deal sandwich and its 1.5g-plus sodium load from the routine.
Flavour has to come from acid and heat: lemon, vinegar, chilli, and black pepper. The palate recalibrates within about two weeks, and the old salt level starts tasting sharp.
Front-of-pack traffic lights are worth checking on jarred and tinned foods. Stock cubes and pasta sauces are frequent sources of hidden sodium.
Keeping the weekly average under the UK guideline of 6g of salt is realistic with home-cooked food. A takeaway-heavy week makes that ceiling very hard to stay under.
Kefir as a supporting dairy habit
Gut bacteria feed into liver fat through the portal vein, and this microbiome route is easy to miss in a diet-and-exercise plan. Endotoxin from a disrupted gut lining reaches the liver directly and fuels the low-grade inflammation that can move simple steatosis toward a worse state.
A daily kefir routine is the cheapest item here once live kefir grains are established. The grains regenerate, and a 200ml glass costs pennies after the culture is running.
The evidence is earlier-stage than the omega-3 or exercise evidence. Fermented dairy belongs here as a supporting habit.
Its plausible role is to diversify the bacterial population and add a fibre-fermenting substrate when it is taken with the lentils and vegetables already in the plan. The practical version is plain unsweetened kefir, either bought from the chilled aisle or fermented at home from grains, taken with the morning meal so it lands on top of overnight fibre. Sweetened kefir drinks with 12g-plus of sugar per serving work against the plan because added sugar feeds the hepatic fat pathway the 12 weeks are trying to reduce.
Vitamin D has a side role
Low vitamin D appears disproportionately in people with fatty liver. It also matters independently for bone density, especially in anyone who has spent years doing little activity and has lost muscle pull on the skeleton.
UK guidance recommends a 10 microgram daily supplement through autumn and winter for the general population, and people with fatty liver have no reason to skip it. The evidence does not support vitamin D as a liver-fat treatment in its own right. Correcting a deficiency is worth doing on its own terms, and the bone-density argument is stronger than the liver argument, so the supplement earns its place beside the main plan.
